Found in the chest at the end of secret corridor in the Chamber of Gorrors. Use it in one of the obelisks on the Isle of Crypt - second obelisk will become teleporter to courtyard, where you'll find Locket of the Tomb needed to enter the Tomb of Astral Dominae.
