Originally may be found in the chest in the Abbey Cellar in New City.
The only map, that is really needed - use it on the Gaelin Stone in the Chamber of Gorrors (on West side) to get Key of Skulls.
It reads:

* LEGEND *
Whatever was written upon 
the map has long ago faded...

Now it is but a blank piece 
of parchment, dotted with 
several scattered holes...

Around the edge of one hole 
near the center, you can 
just barely discern the faint 
remnants of a ring of 
reddish dye...
